(and other forms with Time and Dates)
The eVetsite III System has a great feature for creating forms with selection lists so your clients can select the exact month and day for an appointment or other special event.
To get started create a Forms Page in Edit Menu. Go back to your new page. Click on the Edit Special function button. This will bring up the forms manager set up page. Give the form a name for the email subject line and a target email address where the content will be mailed!

When you're ready for the date fields choose Selection List. The key to making selection lists is to separate each item by a vertical key stroke using the pipe key on your keyboard (the one under the "Backspace" key which creates the "|" type face).
Hint: You can copy and paste the items from this list directly into each selection field:

Continue to add fields as necessary and be sure to click update to save the final form.
